Insurance information

We accept most major insurance plans. The following is a partial list of plans (or preferred provider organizations) whose members may access International Diabetes Center:

• Medica Elect
• Medicare
• Preferred One

Contact our business office by phone at 952-993-3651 to find out what other plans are accepted. If you or a family member change insurance, please notify our business office immediately.

Some plans require a referral, and it is your responsibility to check with your insurance if a referral is required.

Co-payment
Many benefit plans require that you make a co-payment at the time of your visit. You may pay by cash, check, Visa, MasterCard, Discover, or American Express. After your insurance company has paid, you may receive a monthly statement indicating what your financial responsibility is.
